Critical Need Delivery

We recognize some of our families may face critical needs at various points throughout the year. Thanks to the Hazelwood YMCA food pantry and our generous donors, the PTO has a pantry of non-perishable foods and additional resources available to assist families at these times. Families should feel welcome to reach out to the PTO for support by filling out this form. Families may also contact Ms. Evans, our teacher liaison who is available to help. The PTO respects privacy and we keep this information confidential. Teachers may also fill out forms on behalf of families, if privacy is a concern. Due to supply limitations, we must limit this assistance to Greenfield School families only (with at least one student attending our school). Families may fill out the form multiple times at any point throughout the year.
